Who can use this service
Anyone living with diabetes can benefit from working with a Diabetic Educator. We also support carers and family members who want to better understand how to help a loved one manage their condition.
What to expect
At your first appointment, we will discuss your medical history, lifestyle, treatment goals, and daily routine. You’ll receive a personalised diabetes management plan that fits your needs and helps you feel more confident in managing your health.
Follow-up sessions are available to track your progress, adjust your routine, and continue building knowledge and confidence.
Referrals and fees
You may be eligible for Medicare rebates through a Chronic Disease Management Plan, which can be prepared by one of our GPs. Some sessions may be privately billed depending on your referral and eligibility.
