Who can use this service
Any patient with a long-term health condition may benefit from our chronic disease services. If you’ve been diagnosed with one or more of the conditions listed above, we’ll work with you to build a care plan that supports your individual needs.
What to expect
When you visit us for chronic disease care, we’ll take the time to assess your current health, medications, lifestyle, and goals. Together, we’ll create a care plan that may include:
- Regular GP appointments and health reviews
- Medication review and prescription updates
- Nutrition and physical activity planning
- Mental health support or counselling
- Allied health referrals if needed (e.g., physiotherapy, dietetics)
- Education and tools to support self-management
Your plan will be reviewed regularly to help you stay on track and make adjustments as needed.
Medicare support and care plans
If you are eligible, your GP can prepare a Chronic Disease Management (CDM) Plan. This allows you to access Medicare rebates for services like physiotherapy, dietetics, podiatry, and mental health support. Our team can guide you through the process and help you access the care you need with reduced out-of-pocket costs.
